+ Complete PGH MMJ's New Patient Intake Form online
Step two:
Attend your certification appointment with Dr. Rothschild in Pittsburgh's East Liberty neighborhood.

Upon certification, you will immediately receive an email from the Pennsylvania Department of Health with instructions to receive your medical marijuana patient identification card.
